He was the son of Nehemiah and Caroline Curtis of Measham Road, Donisthorpe, Leics., and the husband of Amy Grace Curtis of 44, St Paul’s Road, Smethwick, Staffs. His enlistment documents record the following information. He gave his trade or calling as Policeman, he also stated that he currently belonged to the active Militia. He gave his wife Amy Grace Curtis as his next of kin at 259, Dovercourt Road, Toronto, Canada. His medical examination took place on the 17th December 1915 in Brantford and recorded that he was 6 foot in height, he had a chest measurement of 38 ½ to 41 ½ inches, his complexion was described as fair with grey eyes and light brown hair, it was noted that he had five vaccination marks on his left arm. He gave his religion as Methodist.
